Almost every known risk factor for a small baby moved in the right direction, across four cohorts — 1982, 1993, 2004 and 2015.

So what happened to average birth weight?

  • The counterfactual is a regression, not an observation. Nobody measured a 160 g drop. It is what a multivariable model predicts would have happened to mean birth weight if the known risk factors had stayed at their 1982 levels. Different model choices would shift the exact figure.
  • Four snapshots, not one trajectory. Each cohort is a different set of newborns. The chart shows how the population of babies changed across generations, not how any single pregnancy unfolded.
  • Gestational age was measured differently over time. The 1982 and 1993 cohorts dated pregnancies mainly by last menstrual period; 2004 and 2015 added ultrasound. The authors address this directly, and the preterm rise survives even the most conservative reanalysis — but the precise magnitudes carry that caveat.
  • The caesarean link is an inference, not a controlled experiment. The birth-weight pattern is strongly consistent with scheduled early delivery, but the cohorts can’t separate medically necessary caesareans from elective ones, so the mechanism is argued, not proven.
